Rejected Forms

In the event a vaccination/immunization form is rejected, information regarding why the form was rejected is included in a follow up message. If you need further assistance, please contact the Health Response Call Center (HRCC).

Michigan Medicine (and other OHS supported units) Employees

Occupational and Health Services (OHS) has a separate process from campus for collecting vaccine/immunization information. The information below is related to the most common uses of Enterprise Health for OHS supported employees.

Employees will log in to the Enterprise Health Portal and use their UMICH credentials and Duo Two-Factor Authentication to log in.

Submitting Vaccine Information

Two types of vaccination forms are available in Enterprise Health for students and campus employees:

  • U-M Vaccination History Form
    This form is assigned to all incoming U-M students. Students living in Michigan Housing on the Ann Arbor campus are required to submit it. Instructions for completing the form are available on the U-M Vaccination History Form page.
  • Additional Vaccinations Form
    This form is available to all faculty, staff, and students on the Enterprise Health home page, which is accessed from Vax Viewer. Instructions for completing the form are available on the Additional Vaccinations Form page

Vaccination Documentation

Proof of vaccination is required regardless of which form is completed. However, vaccination documentation must be submitted in a single PDF file or image.
