Vax Viewer is a web front-end to the Qualtrics forms used to collect and view COVID-19 vaccination information. It shows individuals where their vaccination-related documentation stands in the verification process.
Vax Viewer will show the vaccination information or exemption request that has been submitted to the university and the status of those submissions. It will also include links for uploading vaccination information or for requesting a vaccination exemption or postponement.
U-M students, faculty and staff who are submitting new vaccination documents or have questions about the status of documents already submitted should check the Vax Viewer for updates. The system is updated daily by 8 a.m. Information submitted today will show up in the system after 8 a.m. tomorrow.
