Added the following attributes to the My Profile page, EHS Surveillance Details tab:
- Last Form Received (date)
- Last Review Date (date)
- Last All Clear Date (date)
- Form Received Manually (Yes/No)
- Form Received Electronically (Yes/No)
- Form Required Annually (Yes/No)
- Activity list
