Human Subjects Study application (HUM)
- Applications submitted to IRBMED that Request Review by a Non-UM IRB will now be required to answer additional questions related to Certificate of Confidentiality in section 11-2, when applicable.
- The descriptions of Clinical Trial Phases in question 1-2.8.1 have been updated to match the current NCI and ClinicalTrials.gov definitions.
